Congratulations! 

You are ready to graduate! Eligibility for graduation is determined by your accuracy in completing the following information.  Once all requirements are satisfied and this application is received and approved, you will be considered a graduate and will receive your Certificate of Graduation and Credentials.  Exact dates of course module completion, copies of all journals, hours logs, annual renewals, and completion of your CCC must be received by our office for keeping in your permanent student record.

A $50 processing fee is required to review and process the application

IMPORTANT:

LWI no longer require copies of your current medical license(s) and CPR/AED certification to be submitted with your application. However, we do conduct random audits and as a result, that information is required if you are selected during the audit process. We encourage you to keep accurate records of your CE, licenses, and certification(s).
