CCC Project Forms

Once you have formulated an idea for your CCC project, and you have discussed it in brief with your assigned faculty mentor, the next step is to complete the initial project proposal form so we can provide final approval.

      If your project is available in a written format (such as a PowerPoint, Case Study, Handout, etc.) please upload that file here.
    • Your CCC should include a 850 words or less reflection. That should include what worked in your project, what didn’t work, what you would change, and how it impacted your application of MTY as Integrative & Lifestyle Medicine.
      Your work should be presented in a format that would be shareable as a resource for colleagues or patients. The purpose of the CCC is to enrich the community at large and provide a service (karma yoga). We want to be able to share your work and mentored projects with the world at large, so pease make sure it is submitted in its final, shareable format.
      As a participant in the PYT Certification program, you will be, or have already been provided access to numerous resources, including, but not limited to program course materials, processes and programming.Please note that any materials that you receive from, or that are presented at any module, class or training, including, but not limited to, written materials, processes, programming, presentations, graphics, illustrations, charts, graphs, and digital media, whether presented as hard copies or electronically, as well as the LWI name and mark are the intellectual property (“Intellectual Property”) of Ginger Garner and/or LWI and that Ginger Garner and/or LWI own any and all copyrights and other rights regarding the same. Such copyrights and other rights also extend to any Intellectual Property that may be obtained via the internet or any other third party.Any individual or entity desiring to use all or part of the Intellectual Property in any capacity that is not for private use or for use in clinical practice, must first obtain the written consent of Ginger Garner, which consent may be granted or withhold in LWI’s sole discretion. This policy applies to both verbatim use of the Intellectual Property as well as any use of materials that are substantially similar to the Intellectual Property. Any use of the Intellectual Property or any part thereof must be properly cited at all times and at every instance of use. Living Well Institute will require individuals and entities who have been approved to use part or all of the Intellectual Property to display the proper citation.Living Well Institute may, at its sole discretion, grant licenses to a limited number of individuals to use the Intellectual Property to provide Professional Yoga Therapist program instruction and other continuing education programs and/or license the Intellectual Property for more limited purposes.
